Discharge Information

The Discharge Planning Department assists you and your family in maintaining a continuity of care following discharge from the hospital. The discharge plan is a tool used to identify and evaluate your needs in an attempt to connect you with available and appropriate resources needed to maintain as much independence as possible.

SOCIAL SERVICES

If you or your loved ones wants to see a social worker please ask your doctor or nurse to refer you to the social worker. Social work services are available to all patients.
