STATEMENT OF PATIENT RIGHTS AND RESPONSIBILITIES
Thorek Memorial Hospital supports the rights of each patient. We recognize and respect that each patient is an individual with unique healthcare needs and because of the importance of respecting each patient's personal dignity, we provide considerate, respectful care focused upon the patient's individual needs. The hospital affirms the patient's rights to make decisions regarding his/her medical care, including the decision to discontinue treatment, to the extent permitted by law. By exercising your rights, you are assuming certain responsibilities. As a patient at Thorek Hospital and Medical Center you have the right to:
All patients receiving healthcare services or families or guardians of patients judged incompetent possess basic rights and responsibilities. These include the following:
- The right to:
- Receive care without regard to race, religion, sex, sexual orientation, ethnicity, age, or handicap.
- Be treated with dignity, consideration, and respect.
- Have your property treated with respect.
- Voice grievances regarding your treatment or care and to know that your complaint will be investigated and you will not be subjected to discrimination or reprisal for doing so.
- Participate in the planning of your care, and be informed in advance of any changes in your plan of care prior to those changes being initiated.
- Written information regarding advance directives, the right to appoint a surrogate to make care decisions, and the right to refuse care, as legally provided.
- Confidentiality of your clinical records and information regarding your health status and condition.
- Know before the care is initiated the extend to which payment for services rendered is expected from your insurance carrier and at the same time the extent of charges not covered that may be required of you the patient.
- Receive a timely response from the hospital staff to your request for service.
- Receive care from professionally trained personnel and know their names and responsibilities.
- Be free from restraints for acute medical and surgical care.
- Authorize the discontinuation of treatment, which will be in accordance with the policies of Thorek Memorial Hospital.
- Recognize the right that all patients have the right to have their pain assessed and managed.
- Be informed about any research or educational projects affecting your care.
- The responsibility to:
- To remain under a doctor’s care.
- Provide to the best of your knowledge information about your present medications and information about your health care.
- Provide requested insurance and financial information and sign required financial consents and releases.
- Participate in your care by asking questions and expressing concerns as they arise.
- Provide a safe environment in which your care can be given.
- Cooperate with your doctor, our staff, and other caregivers.
- Accept the responsibility for any refusal of treatment or care.
- Treat hospital personnel with respect and consideration.
- Abide by hospital policies, which restrict duties that staff caregivers may perform.
- Abide by city of Chicago and Thorek Hospital’s NO SMOKING POLICY.
- Abide by the Illinois Department of Public Health’s and Thorek Hospital’s policies and regulations regarding isolation and infection control.
